I would like for someone to transcribe the heading and the handwritten notes in the Remarks column.
I can read the other information about his birth but I don't understand the far right column.

just a minor correction

i read in the last colum

out of hospital jun 29 1929

The 10 month day(the mother addr. 10 month before the birth)
because until 1932 that parish is responsible for the care of the child.

the remark colum is also about registration in the public register and church book
the childs birthplace etc.
